To your protocol add:
Unique E tocotrienols taken at a different time than tocopherol.

Resume niacin, it reduces both cholesterol and Lp(a).

Total Cholesterol numbers below 140 increase risk of cancer and increase mortality:

While it can influence the gene expression, it still in can not correct gene mutations.

If I had familial hypercholesterolemia I would try 500 mg of niacin, 3 times a day with meals in addition to your other recommendations.
Also, it is imperative to eliminate sugar, fructose and high fructose corn sweetener (syrup.) This alone could reduce cholesterol by 30% in 3 months.

Niacin lowers Lp(a). Start with 500 mg per day with a meal. You may get a flush. This should go away after you have used niacin for a little while. Niacin also balances the other lipids.

Fish Oil, vitamin D3 and magnesium should be on the supplement list.
Clots are often a result of arrythmias, then sudden death heart attack.
